The
rehabilitation process at Hegau Jugendwerk considers the patient's physical,
emotional, social and vocational needs and requirements. To ensure the success
of the process, Hegau Jugendwerk relies on a team of neurological rehabilitation
specialists, therapists and nurses, who work with the patient and his/her family
to set individualized treatment plans and goals. In addition to providing
specialized medical and neuropsychological care, Hegau Jugendwerk offers a broad
range of therapies, rehabilitative activating nursing and social pedagogical
care.
Upon admittance to the Hegau Jugendwerk, each patient receives a complete diagnostic
work-up, and his/her medical status is established. Then, under the guidance of
the attending physician, the rehabilitation team meets to develop a tailor-made
rehabilitation program for the patient. Depending on the needs and abilities of
the patient, the rehabilitation program may include physical therapy (to improve
movement, prevent or decrease weakness caused by lack of use, manage spasticity
or pain, and maintain a range of motion, gait and balance), activation and
recreational therapy (assistance with activities of daily living, such as
eating, dressing, bathing, using the toilet, handwriting, etc.), swallowing and
speech therapy, music therapy, work and occupational therapy and
neuropedagogical training (or cognitive rehabilitation therapy), which aims to
improve cognitive impairments, such as problems with concentration, attention,
memory and poor judgement. The content, scope and goals of the patient's
rehabilitation treatment program are reviewed and evaluated in regular case
management meetings with the interdisciplinary team and the parents/primary care
providers.
